Managing Diabetes: The CNS Diabetic Program

Managing diabetes can be challenging — especially for individuals who are homebound and without consistent caregiving support. At CNS Home Health, we understand the critical importance of safe and consistent diabetes management. That’s why we offer a specialized Diabetic Program designed to bring expert nursing care directly to the homes of those who need it most.

Through this program, licensed nurses visit daily to:

  • Administer insulin to patients who cannot safely test their blood sugar, draw up insulin, or inject it themselves.
  • Monitor blood glucose levels and adjust care plans as needed.
  • Manage all prescribed medications, ensuring patients are receiving them safely and effectively.

This program is ideal for patients who:

  • Have Medicare or a Medicare Advantage plan
  • Are homebound
  • Are unable to manage their diabetes independently
  • Lack a consistent caregiver to assist with daily diabetes care

Many of our patients reside in assisted living facilities, where our nurses work closely with staff to ensure seamless, coordinated care.
