Community Nursing Services awarded KPI-Excellence by Homecare

Community Nursing Services (CNS) was recognized by Homecare Homebase (HCHB), the leading software provided for home-based care, as a winner of one of its prestigious 2024 Client Success Awards.  These awards recognize HCHB customers who exemplify excellence in quality care, performance innovation and community impact.


The HCHB Client Success Awards recognize clients for results in key performance indications (KPIs.)  CNS was awarded the KPI award for performing in the top of cohort size for Acute Care Hospitalizations, an important metric in post-acute care continuum.  Avoiding expensive and stressful unplanned hospital stays contributes to cost savings and enhances patient satisfaction.  Most patients prefer to heal in the comfort of their own home instead of an impersonal hospital environment.  


Avoiding rehospitalizations takes an “all hands on deck” approach.  Family members, nurses, therapists, certified nursing assistants and others visiting patients at home are constantly observant of any changes that might need treatment to avoid worsening. Communications between care partners is crucial to provide timely interventions so a rush to an Emergency Room can be avoided.  Monica Ebberts, CNS Project Manager, shared that “this award recognizes all the people involved in helping patients heal at home. It takes a whole village!”
